I'm assuming you don't know how to use Cheat Engine, so to keep it simple...
1) Download Cheat Engine (google it) and cheat table from OP, then install Cheat Engine
2) Double click the cheat table, it will open Cheat Engine and the table loads in automatically
3) Run FF12
4) In Cheat Engine go to File > Open Process and select FF12 process from the list
5) In FF12 load a save, level up to your desired level, alt-tab to Cheat Engine
6) Check both HP and MP rows for character(s) you leveled up and input desired HP&MP in the value column (BUT WITHOUT MULTIPLIER APPLIED)
7) Alt-tab back to FF12, change equipment or touch a save crystal and the HP&MP values will update in game
8) Go back to Cheat Engine and uncheck the rows

Basically, you input values from the left table into Cheat Engine and values on the right are perfect HP&MP you should see in game upon touching save crystal.
Guide on how to use this for perfect HP&MP is at the bottom of the post.
Since my game needs an update I guess my previous edits for max hp/mp at level up have been undone. So just to be clear, when using this, Hp and Mp gain are max for all party members? I cant be bothered to edit them myself anymore. (i recall editing all the minimun numbers per level to be the same as the maximum I guess you did the same? ) Tnx!

EDIT2: Editing those values only modifies minimums, but gain range is still the same. So for example if you change 10 to 11. Where you would have previously got 10-15 HP on level up, it would end up being 11-16 after editing it.
