When shooting from the front it becomes even more unreliable. Depending how they are holding their gun, you can hit their arm with a shot that's pretty center mass. Again, any bullet in the game, even poison which penetrates nothing, will pass through the arm and count that as a chest hit. Not the bow or the xbow, though. What should have been a 1 tap is now an arm hit. They live, even though your aim was solid.
It's just how these weapons work. This is why the shot-bolt is so good at ranges you can make it work. Whatever it hits is treated like you put pressed the barrel of a shotgun up against it and pulled the trigger. It will 1 shot even if it hits their foot. And there's no damage fall off, because the range of the "shot" begins wherever the bolt hits.
The shot-bolt also the only bolt or arrow type weapon that can wall bang.
