As for the Fealty, it did take effect for me - screenshot below; and I have multiple (12+) mods installed.

Gaining fealty from positive imperial authority is just a 5% chance every turn (for each elector count). Its not something you are given instantly.
The imperial authority bar even says this if you hover your mouse over it.
yer, I see you get +1 for returned settlements, I did not get it, so It must be somehow bugged.
I had the option to return the land to them, or allow the opposing Empire faction retain that settlement.
So the issue is most likely what you alluded to. It's probably bugged, in that the battle for a settlement and then option to return it post-battle doesn't take effect.
