I have had similar situations happen when I played certain lords. I played as the Slayer King and got some slayer upkeep reduction follower about twenty times.

Do you have anyone around Hell Pit? I know on my Empire playthroughs I'd always get like, 20 of the Estalian dudes when I had an army near that province.

Dunno. I play Skaven. Few turns on. Even after sailing to the high elf continent, my hell pit attendants keep on coming. I cannot make use of all these attendants! Hell is full!

A guess. I do not use Hell pit attendants for my two mains. Tretch and Akorhm. Maybe because I not use them. it give me one each time? The only other hero can use is plague priest. Funny, cos plague priest also win Pet rat, but is unusable for them!

The downside to this is, yes quite a big downside, if you did not catch on, this means I get no other followers. So I have a bunch of heroes without followers, hoping for "messenger" or anything else. But alas, only hell pit attendants - thats it. time for pestilence naves in every province, and plague priests for all these hell attendants

Not all conditions for followers appearing are set up to function entirely correctly, which is especially noticeable with a whole bunch of High Elven followers in particular that will never be spawned save through sea treasure battles. Using one of your examples:

Estalian Diestro is only meant to trigger when a Lord or Hero levels up whilst actually in Estalia, but the code is set up slightly incorrectly so that they appear when a Lord or Hero levels up and you merely OWN some of Estalia.
