How do you pivot Pestilence Breath?

Aftering watching someone cast Pestilence Breath online, I saw them set it's location, then pivot the spell to fire sideways. How is this accomplished? Also, how did I not know this before since PB is in the first title?

if you left click once before casting like any breath attack i'd think.

left click and hold to rotate the the direction for wind spells
