question mark and exclamation mark appearing over monster's head
Any reason, I am seeing a question mark and exclamation mark appearing over monster's head?

Did the monster suddenly reform and wants to give me a quest?

These are the visual effects tied to the Combat System. You can find more details in the Compendium, but essentially you have a chance (and enemies have a chance) to gain a combat effect.

The question mark and explanation mark are universal symbols for quest available/quest hand in.
The people i know as well as myself all got confused with this thinking we accidentally attacked friendly npc's.
It's always scary to see an ettin running towards my goblin with love-hearts above his head :feste:

Not that long, pretty much only in comic books, before that they are literally for exclamation and questions but times change and things become contextual.
In context of games they are quests and hand-ins.

My observation has been the exclamation appears and then the creature attempts to run away, question mark seems to make them pause attacking but continue to move, so not a full stun and the hearts seem to appear when a creature heals itself, as in extra HP and not attraction. All anecdotal.

Question mark is when they are yelling for help..exclamation mark is running away for help...the hearts is them drinking pots
