Having the best weapon you can forge is always a good idea.
Anytime you want is good enough to upgrade.
Choose your armor based on the skills they offer. When you unlock new ones, see if they're better than what you have. Take some time to sift through them and see what combinations of skills you can get.
Mixed sets are almost always ideal all the way to the end of the expansion.
This depends on the weapon you're using. You'll want to be as deep in to the tree as possible. Early game you only have to worry about bone/ore. Bone = damage, Ore = Sharpness.
Elements will be important in High rank and Master rank for fast attacking weapons, bows, and bowguns. Status effects are often worth taking in specific hunts.
Whenever you feel that your hunts are taking longer you'll want to upgrade your weapon
You should be keeping your armour upgraded at all times.
Full sets are only worth it if there is an armour set skill that you want.
In Low rank, wear whatever you want. Skills are all over the place and not strictly needed. Don't ignore them, but read about what they do and look at how the affect your stats.
On first hitting High rank you want to craft the bone armour beta. It's a good all purpose set of skills that'll help you find your feet. Try to get your Attack and Defence skills to max then diversify.
High/Master rank you'll want to focus on skills that benefit your playstyle. Just keep in mind that skills are worth more than stats. 7 points of attack and 7 points of defence will do more to help you than resistances and base armour numbers.
What skills benefit you is as complicated as you want it to be.

As for weapons I just keep upgrading till I reach the end of the tree and pick based on what element and decoration slots I need.
