For example, if you're trying to upgrade a Bone Horn III, you can only upgrade it to a Blazing Horn I or a Hard Bone Horn I since both of those upgrades branches off the same node.
On the other hand, you can't upgrade a Bone Horn III to a Blooming Horn I because those are on different branches, even though they're both in the bone horn tree.
You'd need to either make a new Bone Horn II or roll back your Bone Horn III upgrade in order to go down the Blooming Horn branch.

No, as others have said, just follow the white lines in the upgrade tree. Think you have to progress the story missions sometimes to unlock the next upgrade (don't quote me on this though as I've only played about 10 hours and currently got my twin blades to 3rd bone upgrade).
For example if you have multiple Hammers, there will be more hammer icons further to the right in the "Upgrade Equipment" menu and as you hover over them they will display which of your hammers you are selecting.
So you have to find your bone weapon in the "Upgrade Equipment" menu, select that, then the upgrade tree for it will be usable.
To make the menu easier to navigate, once Ive decided what weapon type/s I want to use, I sell all the other weapon types in my equipment box, then they dont clog up the Upgrade menu.
And if you decide later you want to use a weapon type you sold, the base versions are super cheap to buy/forge anyway.
