Are you sure you are doing the timing and motion right?
Yeah like this guy said. You have to literally be running away from the monster like.. " Oh ♥♥♥♥! Run man run!!!! ". Think of it like in the movies when the heroes are running away from a ticking bomb and the explosion happens. The bomb is the monster and you the heroes. Gotta be running for yer' skins for the dive to work. I typically like to have my camera always facing the monster ( without that stupid lock on. ) and it never fails to work for me.
For example the large sweeping beam attacks that several monsters do usually has them putting their head to the right or left then sweeping across. I think that maybe you may aim your back towards the monsters head (since that's where the attack is coming from) but the actual place you should be aiming is the center of mass of the monsters body.
Though I'm not entirely sure about this.
You can see you character run in " oh ♥♥♥♥" style before you can dive. If your character run like that then you will dive if you dodge away from monster.
