Yes, the game gets regular patches, there'll be another one in a few days as prep for the Lunastra event that starts Thursday/Friday depending on your timezone.
Main thing to be aware of is it's a grindy game. If you don't like grinds, might want to look elsewhere.
The main long-standing bug would be low-res textures. I honestly never notice it.
The game is regularly updated and patched multiple times a month.
You can find plenty of mods on the Nexus. Mods are not officially supported, and Capcom has basically said "Do what you want, you won't be banned, and we won't fix your game if you break it".
Any downsides or issues the game may have are minor at worst. As long as you don't have a potato for a PC, it'll run just fine. I think it's a great game and totally worth every cent, and that's before the fine sale that's currently going on.
Things that have been fixed: heavy fps drops while fighting some monsters or using some kind of weapons, frequent connection issues, bad matchmaking, missing graphical elements or low res textures (still not entirely fixed), bad mouse controls, missing keyboard or graphical options.
The game does receive regular patches and updates (more than any other Capcom game released on PC) and the devs are seemingly pretty attentive to the issues of PC players.
Capcom doesn't officially support mods, however the modding community is pretty active and they've been doing a pretty good job so far in addressing some of the main QoL issues.
Be aware that the game is currently a few patches behind consoles but we're close to catching up.
currently: minor disconnection issues, nothing big, from personal experince: once every dozen hunts or so, better then it used to be
some graphical issues have yet to be fixed, again, minor, but looking at armor is just ugh for the time being, its like looking through smudged glasses.
the most major issue i can think of is the games threading, and therefore the game, is horribly optimized, having like a constant ~150ish running, which makes it eat cpu that it does not need to eat. thankfully there is the (special k? i believe, someone correct me please) mod, that fixes this and brings it down to a more reasonable level.
as for mods, there is plenty, im not sure if there is any HUD mods as i do not use any myself, but i know there is plenty to help negate the pointless rng added to world that wasn't in older monhuns, so if you grow tired of arbitary systems like that, they exist, they're on nexusmod as others have said.
overall; a good grind game, and the port is getting better, but its slowly doing so, right now it seems to be in an okay spot, i wouldn't reccomend it for full price just yet, but on its current sale? its not a bad deal.
