Error 12 is quite a popular topic ever since launch I believe stretching across many users with various configurations and settings with exception of consoles ofc.
Try DDU to do a clean uninstall of the display drivers, then reinstall your drivers which ever brand it is.
Do make sure windows is up to date as well.
Lastly do close unused programs that are running if you don't need them.
From experience DDU usually fixes most problems if it is display driver related.
Additional information: The display device seems to be the "Intel(R) UHD Graphics 620", whatever that means.
The best thing you could probably do is go into the intel control panel and create a custom resolution lower than the standard resolutions the game lists. Try 960x540.

pending on your specs your GPU should be fine in any case and a minimum is recommended to be something like a 960 or 970 GTX worth of power. most integrated laptop ones end up being trash unless you bought a 2000$ laptop focused on the GPU and CPU being mighty.
it seems the problem was brought back with a recent patch (witcher) from ppl experiencing the same error recently.Luckily it rarely happens to me (twice so far)

1080ti (driver 416.94 clean installed with DDUCleaner suggested by greybear from the nvidia forums as one of the most stable drivers)
9900k 4.7ghz
ddr 16gb ram
970 evo 500gb
My rig is fairly debloated
No overlays
All "game" settings turned off
Ansel and nvidia telemetry off
And no bloat apps running at the same time except discord/steam.
I run the game with fullscreen optimization disabled.
i dont think drivers are an issue here anymore, they used to be for performance but now not so much.
theres gotta be something we're missing.
edit: whats interesting in that log is it posted so many times before finally recovering... if i had to hazard a guess there might be something wrong elsewhere in the setup either hardware or software. i'd try running lots of maintenence stuff like system file checker and the like and see if they find any problems as well.
I tried system DISM.exe /Online /Cleanup-image /Restorehealth and sfc /scannow just incase and nothing was found.
