Oh I am never going to play with others while doing it. But, I know the game, I've been playing Monster Hunter since MHFU on PSP, and put roughly 400 hours into MHW alone, so it's not like me skipping content now will affect my end game performance. I will of course run fights legit solo before joining up with others for end game though.
As for "how" they could deal with cheaters, there could already be a system in place that keeps track of things such as modified stats, items in shops that shouldn't be there and loot drops that shouldn't happen outside the rotational event timeframe. Capcom flips the switch, repeat offenders accounts are flagged and for simplicity sake all the identified cheaters names change to a different color for anyone online to see. They could even add an option to hide your lobby and SoS from flagged cheaters as well as not see theirs on the list that would be default turned on so that you can only play with cheaters if you actively want to.

Oh that is fantastic! I had no idea this existed. How risky is getting items I wouldn't have ingame access to yet?
I'm not sure, but it should be fine. The game only prevents you access by hiding and stopping the ability to craft/buy. It doesn't do any inventory checks to make sure you're allowed something.
If you're concerned try using cheat engine auto/instant quest complete hack and rush the story. Should unlock all items that way.
I'm not sure if there's a way to skip cutscenes in cheat engine, but that would be a nice one to look for.
