OBS Preview will always be whatever you have your output set to .... Sice twitch does not support 120 fps streams i would assume you are streaming at 30 or 60 fps...

ok thanks ... So you are say using gsync at 120 fps but outputting OBS at 60 fps and the stream lags..

That will happen Windows Display Manager does not handle mixed refresh rates well so 120 fps gsync out to 60 fps stream = going to lag.. Especally if you have a second display.. It's a windows problem not a obs problem ..

Fix run 60 hz across the board while streaming ... One day maybe MS can be ased to fix the WDM to allow for mixed refresh rate.. Or run 2 monitors at 120hz or 144hz and then obs out to 60 it wont lag..

It's also caused in windows 10 by the gpu being maxed it cant handle the preview on obs when the game is using 100% of the gpu. All thanks to wdm again as it don't happen in windows 7 and 8.1. You have to frame lock the game down to 60fps if it running over 60

TBH easy work around I do it all the time for streaming , use 2pc's If you cannot use two pc's just get a game capture card run the second video output to it or third then clone whatever display you want for gaming to it.. Fixes the whole capture lagg issue.

i use a black magic capture card to a second pc so am fine lol, let the second pc with a i7 do all the processing then game does not get touched

Try switching encoding from software to hardware, that might also fix your issue.

Using vsync should fix your issue, however, you will also play at lower frame rates.
