A headshot with an AWM is a knock/kill regardless of desync; I'm not sure how that is even relevant.
The only way it might appear to be to take 2 shots is if you hit them in the neck, since shooting to the neck also does helmet damage. This makes it look like you've gotten a headshot since the helmet breaks but isn't actually a killing blow.
