What are the conditions to be able to transfer ships between fleets?

For Chaos you can change between any Legion and Renegade fleets. The ships change designation.

You cannot change between either of those and Marked fleets.

For the Imperium you can transfer ships only within their type. So Navy, Mechanicus and Astartes can transfer ships within their fleets, but not to another. So a Navy ship can'y be transferred to a Astartes fleet.

Necron and Tyranids have no restrictions.

Flagships cannot be transferred in any case.

flagships can be technically be transfered as you can promote another line ship to be the flagship

Right, but you can't do it through the transfer interface.

Also, if not obvious, both fleets need to be in the same planetary system for the transfer to be allowed, and there has to be available room in the receiving fleet for the ship to bring over.

It also reduces fleet movement to 0...but you can switch as many times as you want at 0 movement points.
