How do you unlock "Followed" and "Wake Up" achievement?

Level 13, in a particular house, in a room upstairs to the left, there is a doll on the bed, and a deer head on the wall. After defeating the waves of enemies to clear the house, stare for a while at the doll, then the deer. I found the deer harder to register. You'll hear a sound if you've done it right in each case. Keep a save file for the house as a backup in case the Achievements don't pop after exiting the game from here.

I found it. So it's the second house after you go through the underwater area. I had to jump on the deer's head and look down to unlock that one, for anyone who is wondering. Thank you.
