While both worlds are set in realms where eldritch horrors are real and more or less default,
The Healing Church is eldritch horror worshippers pretending to be bible readers to spread their influence in a world that is already doomed. They're like quack doctors or fake authorities giving fake emergency measures during the Pompeii volcano eruption because their real goal is to try and get as many people killed as possible while pretending to do good. Pure evil.
On the other hand, Gwyn combated and actually succeeded in banishing the eldritch horror-esque nature of the world and establish its own sub-domain where things are better overall... at least for a time. The problem Gwyn now face is that no one can beat nature. The laws of equilibrium and entropy would eventually prevail and gradually reverse the world back to its original state, that of its default full eldritch horror state. Realising this Gwyn completely lost his marbles and desperately try to maintain the orderly world he created and in doing so he did a LOT of atrocities to try and maintain control which is bad, sure, but then again, the cards are completely stacked against ANY AND EVERYONE in this world in the first place. Gwyn at least put his word where his mouth is and ALSO threw himself into the fire and his crispy fried husk still stood to guard the First Flame.
The sin of Healing Church is that they did everything wrong and exacerbated horrors and miseries in a world that is already screwed in the first place.
The sin of Gwyn on the other hand, is just the unfortunate fact that he existed in a world that has absolutely no hope, even down to a molecular level.
Gwyn bashers often forget that the default state of the world of Dark Souls is an endless field of writhing ashes that are neither alive nor dead like a very nihilist afterlife. And that's on a good day. On a bad day it's Oolacile, filled with madness and terror... and endless fields of writing ashes. No wonder he tried to change it because it sucked so bad.
If anything, Healing Church despite their outward false appearance, their real goal is more akin to the Darkwraiths in Dark Souls, really.
No matter which you choose, you can get either ending.

Then again, seems kinda moot since you're ultimately going to be doing a favour to one of the serpents when you choose the ending. All it does is locking yourself out of the serpent's services.

Even if you were to quit the game and go "well the best move is to not play" and go sit on a tree log like Crestfallen Warrior at Firelink Shrine, it just means your character does nothing and eventually goes hollow. What's more, someone else will take your place to do one of the serpents' bidding. Since to no one's surprise, the player character isn't actually "chosen" or special in any way. You're just a piece of meat that manages to kill your way across town to run errands; if you fail, someone else will take your place sooner or later.

Then when I needed him for discarding unwanted items, he was always sleeping. Now on my second playthrough, I find this Kaathe character, and I come to find out he's more buff than frampt and more of a primordial serpent than that sleeping pile of ropes. SO... all that said, I'd go with Kaathe lol.
