Knight's Honor Achievement not popping

I have tried dropping all items (4 at a time), using the chest, and I am 100% certain I have all the gear from the wikidot. I am currently on NG++, and this is just the one thing that is seriously infuriating me.

If anyone knows of what I'm missing or has experienced same issue/s, help would be greatly appreciated.

It is the most problematic of the achievements

My only caution is gathering the items you must always be online FOR STEAM.
The PVP online is separate since the great foobar. One assumes that it is truly separate as they are no longer connected, as far as I've been informed.

It does appear like you have all the things...

The achievement does work, from what I've read online most people suggest dropping each item on the ground then pick it back up, I've also read upgrading a boss weapon might do it... I dunno.

I don't know if it'd be problematic but for me I didn't upgrade any of the weapons at all...

no i upgraded some of this weapon and 1 days ago i completed the game and gave me achievement

i don't see in your list the "Jagged ghost blade" dropped by ghosts in new londo, hope this will help you

are you sure? he should try with that, if i remember well the jagged one was necessary for the achievement, but i dont know, maybe you are right.

I got the achievement only a few weeks ago and im 100% sure you don't need the ghost blades.

you are missing obsidian greatsword (Kalameet tail), which should be under moonlight greatsword!

I got the achievement and i never even killed this boss. pretty sure dlcs dont count for the achievements.

I didn’t see any of these I must’ve had notifications off, sorry for that. As of current I haven’t had a chance to play but I’m thinking of experimenting with another playthrough and going through it all again to see if the achievement pops, otherwise if it doesn’t I might try deleting and reinstalling who knows?
