If any game doesn't see my controller for any reason I just use DS4. No, not Dark Souls 4, it's called DS4Windows and it can turn your controller into an xbox one. After it recognized your controller go into settings and toggle "Hide DS4 Controller" on and then accept the pop up and it should work ingame. Most of the time you don't even need to restart the game.
If not, try this in game: Ctrl-Alt-Del will bring up the Task Manager. Once up, cancel out of it. That might fix it.
If you want to stick with Steam use Ctrl-Alt-Esc. Same solution, cancel out, see if it fixes it

The most stress-free workaround for DS:R is to just use Steam-Input; Right-Click this game in your library --> Properties --> Controller --> "Enable Steam-Input". But there's a caveat, since it will just emulate your DS4 as an XBOX controller and that is, that the button prompts will show XBOX buttons. There should be texture-replacers on nexusmods.com though and as long as they only touch those button prompts, these mods should be safe, but still use them at your own risk.
