I don't have a screenshot and I am using a cord.
dont use bluetooth, you gotta use cord if you want to have the ps4 button prompts buddy. Also make sure to follow Seer's instructions.
There is mod on Nexus for PS buttons promts:

turning the steam input per game to force off just turns off all controller options for the game...wont let me use the controller at all...
1. Quit DS4Windows
2. This step is NOT done in big picture mode. Navigate to your Steam Library, find Dark Souls Remastered, right click it and then click on "Properties". Click on the "Controller" tab, and on the next menu you should see a drop down list on the right of it that gives 3 options. Select "Disable Steam Input" in that list.
3. Click on Dark Souls Remastered on your library where you would click the green play button, and on the right you'll see a controller icon. Click it then make sure that "Disable Steam Input" is also set to off there.
For some reason, setting it to off in the menu I mentioned in step 2 does NOT set it off in the menu I mentioned in step 3. This was what was causing my issue, and now my bluetooth PS4 controller works perfectly with the PS prompts we've come to know and love.
