Weird reset glitch?

I seem to have encountered a really strange glitch/bug. I've already completed everything except the Painted World, Asylum 2.0 and the DLC. To clarify I have not even entered these areas.

I started playing today and went to farm chunks in New Londo Ruins, but I noticed that all items that I've already picked up are there again. Firelink Shrine chest gave me key items that I have not picked up, like the Peculiar Doll, Magic and Crystal embers and the Purple Coward's Crystal. Now I am in Darkroot Garden and same thing happened here. Items have respawned and I have to lit the bonfires again.

Has this happened to anyone else?

EDIT: Leather and Knight sets had respawned but not the Grass Crest Shield.

EDIT 2: It seems I cannot even go hollow after dying.

I got the same bug. And on top of it, I can't visit the painted world, because the cell with the doll is locked and cannot be opened.

just happened to me as well just started playing, and played till i got to the black smith before finally closing the game, reopened the game and i was in a different location then when i closed and items where reset. when i went back to firelink shrine a chest there gave me all the lord souls and a bunch of other items
