You had hp bars researched right? The only thing I can think of is you kept forgetting to let them regenerate in the main rooms or you are sending someone with low hp. What other abnormalities did you have?
If you had the armor abnormality did the employee have a blue/orange/red aura around their head. If they did read the tips on the armor looking abnormality. That's the only other thing I can think of that could have caused this.
I am certain that the employees were at full, or near full health each time. I was carefully monitoring this as I was specifically trying to figure out what was happening to them. Also, every time the death occurred pretty much as soon as they entered the room, before the abnormality would have had a chance to damage them. And no, I did not have an armor abnormality.
As for what abnormalities I did have, let me think. One Sin, Beauty and Beast, Bathtub, Cleaning Robot, Forsaken Murderer, Fairies (and no, the deaths weren't caused by them; that was the very first thing I checked), Heart thing (buffed max health), and Sap. Might be missing one or two; it's been a few weeks now.
One feature I wish the game had is a post-mortem report detailing exactly what happened to deceased employees. That would make nailing down stuff like this easier and be a fun read.
