What's missing from my codex?

Hello, I'm almost through the end of the game and want to make sure I reach it with a 100%, however, after getting everything but Don't Touch Me and Bald is Awesome, it says I still have only 98%. Do I need to get the joke abnormalities as well?

As far as i remember, the following are optional and not required for a 100% codex:
-Apocalypse Bird
-Bald Is Awesome
-Any uh, community abnormalities (those come in round containers when you select 1 out of 3).

So i guess you probably missed one or two...? Or didn't let a certain abnormality transform...?
Or perhaps you need to unlock Don't Touch Me's name? (I'd recommend doing so by attempting to select it in the preperation screen where you can hire employees, until it does it's "crash", as to prevent any actual possible loss of stuff.
Did you ever do the tutorial? I wonder if you need to do that in order to complete the codex, hm...

That's the best i can come up with so far. Or look on the Wiki to see what abnormalities are in the game, and see which one you can't find in your codex.

I already had WhiteNight and read through the wiki with my codex by the side, so I guess I'll just have to try and get Don't Touch Me again then and get it's name this time

Bald is Awesome is required for the 100% codex, don't touch me from my test is not required
Also Don't Touch Me can not get level 4 observation, it will stop at 1 upon crashing the game.
