In all seriousness you need an employee with 140 Temperance + all her observation bonuses in order to easily get 16 PE-boxes when her Qliphoth Counter drops, so its no surprise that someone is struggling to get the 16 PE-boxes she needs to increase her Qliphoth Counter. The 2 ways to prevent her from decreasing the Qliphoth Counter is by either getting 16 PE-boxes and then pass a Meltdown or the Simple way of kill 3 randoms.
For her escape, Uh well, you already know that the stupid clerks can't survive, so all the escape on Death abnormalities are going to breach. Best to just reset if you can't deal with those. But other then that, her escape is kinda straight forward, I think. She telegraphs both of her attacks(2 of them) pretty well, but just in case sure everyone has Black resistance(0.6 or better) since her Teleport to an employee/clerk deals damage to everything in the hit box (I forgot how big it is).
I will leave it to someone else if I missed anything along with clarity on specific aspects of her breach since its been a while.
Dispose of 3 clerks each meltdown with this ability and she'll never go into a breakdown.
It's that or just pray to friggin' God that you'll succeed in 16 boxes or more. Even the most powerful employees can screw it up, i know from firsthand experience.
But if you got employees that strong - she's a piece of cake to supress and any ALEPH-tier abnormality (with maybe one or two exceptions) can be supressed, too.
-If she does break out, you of course should position people so they are not stuck in a long hallway, and can escape the laser by going up/down easily.
-She'll teleport onto any employees, preferably/exclusively your employees it seems, which does black damage as she teleports, so just make sure no-one's super weak to that.
-If she's being attacked while damaging stuff (or just employees) with the beam, she'll heal faster then her health drops. No need to worry about clerks though, they die very fast.
One strategy, is to maybe, if she's having a breakdown, is to put all your employees (except whoever works on her) in one location, possibly an elevator shaft, black-shield them as she transforms and as she finishes her beam attacks (she's likely to teleport at those moments), wait for her to teleport to them, and just beat her up with all your forces. Should be a short fight.
don't chase her as she teleports a lot and will take you forever to take her down
