T-Shirt Achievement
It relates to getting 100% in the game. Given that the Heskel and Brutalizer Twins achievements didn't pop for everyone that's beaten them, I'm also assuming I've need to do another full run, or I've missed something.

Killed/Secrets 100%, found every mega secret as well as the secret level, and it says (or is it...?) at the end of both attempts on UV and MF prior to 1.02 patch.

UPDATE
It is indeed bugged. You have to get 100% kills, 100% secrets, find all mega-secrets (all of which are known and documented) and beat the secret level (regardless of difficulty).

Workaround
Just get to the end of your play through, right before you kill the final boss, open the command console (shift+~) and type "setvar yahahayoufoundme2 1" without quotes. Hit enter, kill boss, win achievements. May require multiple attempts to properly unlock. If after or during credits it doesn't, reload the save, re-enter the command and try again.

FINAL UPDATE
It's fixed, and unlocks as intended.

Mine says something like "So close..." I'm chalking it up to missing one enemy in the last area but maybe I'm totally off and I need to 100% the entire thing ... in which case I'm boned lol.

"So Close..." means nothing. That's just Steam being Steam; it exists on all achievements for all games when viewed from the overlay summary.

Is there a way to keep track of the stuff that you done for this achievement or you don't know until you finished the game?

At the end of each "zone" it will let you know how many secrets and kills you have. So if you've missed anything you could always go back.

I did another full 100% run, didn't pop. Up until the final completion screen, I had found every secret, mega secret and secret level. Unless there's one hiding in the final zone, and nothing shows up for finding the secret slime world multiple times, I'm a bit stumped.

I've reset my achievements and going through again. I have a guess that maybe its a similar issue with a couple other achievements not popping until you do them again. It's already completed, but won't unlock due to everything having been done, and nothing is triggering it.

There is a "non-secret" secret area in the game.

A secret area that is not counted in the levels secret count.
It's also tied to a specific achievement.

The area with the switches 0,2,3, right?

That does lead to a secret, specifically a mega-secret. Mega-secrets don't count towards actual secrets, but are an additional find towards completion.

On the topic of:

Inside the code for the game, it does list finding every mega-secret to be included in the T-Shirt achievement. Weird thing is when you beat the final zone, it says the message as if you didn't find that zone's secret (or something), which is thoroughly perplexing.

I also went through the whole game again but on the easiest difficulty setting. I killed every enemy and foud all secrets and mega secrets. I also went to the secret slime world 10 times just to be sure (but nothing changed). I guess there are more things tied to this achievement than just killing/finding everything.
I know there are more hidden messages/references outside of the actual gameworld that are only accessible through cheats like no_clip but I honestly dont think these are included here.

PS: I also can confirm that cheats do not spoil achievements. A friend of mine unlocked the Resident Evil related achievement by using no_clip and it popped up for him.

My reigning theory is it's bugged if you've unlocked everything simultaneously, and verifying it now.

You can also unlock the resident evil achievement when you return to the level, I've seen a couple people discussing it that it doesn't work, but they're attempting it on the first time through.

The bonus mission is the preview campaign, which I've played to 100% prior to release, but it couldn't hurt again after I finish this run through of the main campaign.
