aw, that stinks.
I've reset my achievements and going through again. I have a guess that maybe its a similar issue with a couple other achievements not popping until you do them again. It's already completed, but won't unlock due to everything having been done, and nothing is triggering it.
A secret area that is not counted in the levels secret count.
It's also tied to a specific achievement.
I think I know that one and I'm trying really fricking hard it figure out out (Don't tell me or give me hints!).
The area with the switches 0,2,3, right?
On the topic of:
Inside the code for the game, it does list finding every mega-secret to be included in the T-Shirt achievement. Weird thing is when you beat the final zone, it says the message as if you didn't find that zone's secret (or something), which is thoroughly perplexing.
I know there are more hidden messages/references outside of the actual gameworld that are only accessible through cheats like no_clip but I honestly dont think these are included here.
PS: I also can confirm that cheats do not spoil achievements. A friend of mine unlocked the Resident Evil related achievement by using no_clip and it popped up for him.
You can also unlock the resident evil achievement when you return to the level, I've seen a couple people discussing it that it doesn't work, but they're attempting it on the first time through.
Your 100% includes the bonus mission "Crisis in Columbia"?
