What items carry through the time skip?
For the time jump from Medieval to modern times, what items should I have in my inventory to keep them?

1) Do you regain the content of your storage chest (from the Vampire sanctuary under the university / church)?
2) What about items in your comanions' inventory? Should I put everything onto my main character before the time skip?
3) Do I keep my money?
4) What items exactly do transfer?

It's been awhile since I played, but pretty much anything with blue text (especially unique items), including discipline and blood jewelry and amulets, that will be scattered around the Society of Leopold. If you have any unread skill books, make sure you use them before the fight where you run into ghoul Aneska... as they will not carry over.

Yes, I just tried it and according to my experiences, what carries over is:
- Christof's currently equipped mainhand weapon, whatever it is
- any "blue" (magic) accessories like Gloves of Strength, Blood Rings etc.
=> But only if they're in Christof's inventory or currently equipped by him.
The things that carry over could all be found in the very first room you get revived in.

What didn't carry over for me:
- any armor, even if its blue (makes sense - you couldn't run around London in medieval plate)
- my shield
- any accessories that are not magic (like the +Appearance rings)
- anything in my companions' inventories or equipped by them
- anything in the storage chest

So if you want to carry as much over as possible, put all magic (blue) accessories in Christof's inventory and equip him with your favourite weapon. Don't bother about armor.

You are correct. Items you want to carry over into modern times do need to be stored in Christof's stashed inventory as well as on his person. Armors that do carry over are blue gauntlets with buffs, blue heart shields and shields with resists, and - if I'm not mistaken - blue Unholy armors are as well. Unfortunately I just checked my saved games from a 2009 install for Windows 7, and none of them are in modern times so I cannot verify that about the Unholy armors.

Other things that will carry over are blue vitae bottles (if I remember right, you won't find them in modern nights); blue unique items (such as the Ainkurn Sword, Ivory Bow, Argent Baton, Beserker Fang, and any non-human only weapons you can find around the game).

Just remember to make sure everything is stored in Christof's inventory before the conversation with Vukodlak and Anezka in Vysehrad Castle. If it helps, access your stash via the console and strip everything off yourself and your coterie - including vitae, and put them in your stash. I can pretty much deck out an entire coterie for modern nights beforehand if my search is thorough while inside the Society of Leopold, especially if I have stripped Erik of blue items before he meets his fate at the hands of Etrius in the Haus De Hexe.
