Died, game thinks im alive, can't reinforce

After getting blown up by something in the current sneak attack event, the game won't let me reinforce even though my corpse is blown into several pieces and I have zero hp.

Enemies would also gather around the dead body and try to melee attack it until something else got their attention.

Below the first image is another one in a different mission and campaign where I apparently got hurt against bugs (not sure how because there weren't any around when it happened) and my character kept complaining of fatal injuries, but I couldn't stim myself until I actually got injured again. There was just a notification to stim myself, and my stamina would not recharge.
