The notable thing is that no weapons are missing so they show up ontop of the weapon that they should have been a placeholder for if the model had been missing?
It's definitely a weird one; for a while I thought I was exclusive to the training until it happened to me once in an older patch in a regular mission.

This. I used to see exclamation points for the same reason in Fallout New Vegas.
