Report a translation problem
I and a few others are having this same exact issue in my friend group. All of our names are within the required character limit. Some of us have been able to accept friend requests, some of us have not. Some of us can send invites and the other sees it pending, but can't accept it and it stays pending forever.
This needs to be a top-priority fix.
The problem does not seem to be with the Steam friends list but exclusively with the PS friends list. By PS friends list I mean the friends list in Helldivers II under the social tab in the options.
It is currently not possible to manage the PS friends list because accepting a friend request via the friend code does not result in a response. If a player without a working PS friends list tries to join a Steam friend (SHIFT+TAB ingame to start Steam Overlay), they will receive an error stating that they cannot join the game because the lobby is private.
However, it is possible to join a game via Steam without a working PS friends list as long as the host has set privacy to public and crossplay to active in the Helldivers options. The host's settings are crucial for clients whose PS friends list is not working.
This does not fix the PS friends list bug, but is a workaround for anyone trying to join a game via the Steam friends list.

Exactly the same happened to me.
Friend requests that I make under "Social" by entering the TAB key and the friend code have no effect, regardless of whether I or others see and accept the friend requests.
Because of this, you can only join ingame sessions of players who are already on the friends list, but not sessions of people who are not yet on the friends list.
However, there is the option for PC players to join the sessions of other PC players via the Steam overlay (SHIFT-TAB). To do this, both PC players must have Steam and be on the friends list on Steam and each have the game Helldivers II installed. The Steam friends list is independent of the game. This friends list on Steam works perfectly and other PC players can also be added there.
What is not possible due to the broken ingame friends list is adding PS players, because they do not play via Steam. A patch is needed. But for PC players who want to play with other PC players, there is the workaround described above.
In order for PC players to be able to join via Steam, privacy must be set to public and crossplay to "on" in the host options ingame.
