Stratagems choosing the wrong one
I was trying to call in my support weapon but it would choose a different one every time. I would dial the laser canon and it would put the mortar sentry in my hand. If the sentry wasn't available it would give me the 380mm eagle then the rocket pod eagle. I had to expend everything to finally be able to dial the laser canon.

On automaton planets there's a chance for the modifier called "Electronic Countermeasures" which randomizes which stratogem you actually get, just pay attention when you type it in the make sure you get the right one before throwing. It's normally easier to put the wrong stratogem in to get the one you want.

To add on, at the mission/operation select screen there's a tab called "Effects"; open it up and it'll tell you what to expect, from mission stratagems, to debuffs (overheating, cold, mixed up codes), and etc.

1. If this is intended, it should be more obvious because it just seems like a bug if you don't know where to find that info. My 3 friends playing with me also thought it was a bug.
2. I don't think it's interesting or fun, it's just a hindrance with no engaging or creative workaround. I did see the heat and cold modifiers and those are fine since you can tailor your loadout.

RTFM. Seriously. Read the conditions for the planet before each mission. Also, after you call something in, you can look at what you got in your hand, and cancel if it isn't the right one, so you only lose the few seconds, but not the call ins.

Yeah i'm just here to chill and play the game... I'm not going to read everything about every mission. I shoot enemies and go the the orange icons. The fact that multiple people, not just me and my friends, have not seen the modifier suggests that the UI needs adjustment.

Also suggesting I read when its in my hand would only work if I knew about the modifier beforehand. I did do that to figure out what I was throwing but thats not the original issue I had.

On every planet bottom right it says effects, these could be rain, fog, spores or this, you dont look then that is on you, these effects could make your stratagems take twice as long to refresh and such meaning if you have a 360 second cooldown on one it becomes 720 seconds, rather dumb to not look.
