This one isn't very polished so I shall respond as if it was real.

An operation contains 1~3 missions, depending on difficulty. 1-2 have a single mission, 3-4 have two missions, and any more have three. The amount of missions is equal to the amount of times you can earn medals from a mission within this operation, and the amount of medals per mission depends on difficulty.
If you fail any mission, the operation is failed and you can't access it.
To obtain medal rewards, complete any mission in the operation. This earns you the first reward.
To obtain the second reward, clear another mission in the same operation. To obtain the third, do so again.
Completing an operation also awards progress in the Galactic War - this is not on a per-mission basis.
