It doesn't seem like it's one specific item that's missing a file or anything. The question mark may appear as the actual thing for other players on the team.

For example, part of my grenade pistol was appearing as a question mark for one of my friends even though it looked normal for me.

It means you're missing a file. It's a filler for a missing in-game asset.

Verify your game cache.

Steam Library > Helldivers 2 (RMB) > Properties > Installed Files > Verify integrity of game cache

Wait for it to finish and see how many files you are missing.

It depends on the file missing and there is no consistency that I saw with what asset in particular is missing. The first time it happened to me, it was an turret.

The second time it was an explosive canister.

Important thing is to get the files retrieved in case it causes performance issues or crashes in your lobbies.

i verified files and there were 3 missing ones, i thought this questionmark was a placeholder for a missing texture like the interior of the grenade pistol

Yeah even with verifying multiple times after each play session or update I still get this artifacting on the Grenade Pistol which my guess it is a known bug.

Its a place holder IF a missing asset is present. You were missing assets.

It's not a permanent place holder, just one that is put there until you retrieve your files.

Note that reacquiring files before each log in is almost mandatory for HD2. I do not know the cause and reinstallations seem to not help anybody.

I can say with certainty that there seems to be a major correlation between Task manager / ALT+F4 closing the game if it doesn't shut down correctly that seems to be causing it though. Seems like whenever I have to do that I am always missing 2-3 files so I think at the very least a .config file goes wonky since the game isn't exiting out and saving properly.
