That which does not kill you
how do I get injured in all my limbs and stay alive for the achievement to activate?
when all my limbs are injured i'm either dead, or the bugs 1 hit me.
is there any trick?

If I had to guess the trick is the "Democracy Protects" armor perk which has a 50% chance to leave you with 1hp on a fatal hit.

Use that and drop a 500kg bomb on your head while flipping a coin is what I would do.

Yeah i just randomly survived a bomb that fell on my head with the armor perk

This also works if you just stand on the standard grenade
