Blue hexagon not visible in inventory while I already collected it..?

As you can see from my screenshot, blue hexagon shows up in map but when I look into inventory it still says I don't have it.

Same here: it appeared in my inventory again when I re-collected it after its boss, but then it soon disappeared again. (I forget if immediately or after I slept to change from day to night and then back again.)

I headed back to the boss fight through a teleport after beating the game. It was still on the altar. I did cheese the boss fight so it may have something to do with it.

Same happened to me^^ Does not seem to hinder progression in any way I noticed. and if you return to the pickup location later you can pick it up again, which also seems to break and fix nothing. Seems to be just a visual bug in the menu luckily.

Same here. Pretty benign but common bug

For the record, I also encountered the bug. Every time the game was started the key would be missing from inventory display but could be picked up again from its original location, not that it seemed to make any difference to anything.

I've completed everything you possibly can in a first run of the game (with the exception of beating the final boss) and the blue key isn't in my inventory. When I go back to where I got it, it's visibly still there on its little pedestal thing. Not affecting my progression in any way.

Same for me, luckily I was able to beat the game without any issues
