Better question, why spend real money when fake money is so ridiculously easy to earn?
Someone correct me if I'm wrong, but I'm pretty sure there's nothing in the store whatsoever that you can ONLY purchase with actual currency.

I could be wrong but I think you are right. Thats why im so confused about the micro transaction controversey. I dont see anything wrong with microtransactions as long as they do not lock content that should be part of the main game and do not give players advanatges in a competetive enviornment.
The prestige weapon/vehicle skins can only be bought by the silver bars (the MT cash). You can find some of it ingame inside safes tho but I doubt you'll get enough of them to buy everything.
Edit: The only thing worth getting anyway is that smiley face shovel!
I also don't get the microtransaction controversy. If this was a mmo or something I would see the issue, but it's single player. What advantage are they gaining over others? Really, you only need to pay if you want everything right away. You can get it all naturally by playing the game, but you have a choice. Choice is always good.
