If that's not helpful then look into Steam commands you can enter before starting the game. Might be something there, sorry I don't know more.

Hmmm, alright thank you so much. I'm going to look into it further then. :]
1) Turn On nVidia - Fast Sync, AMD - Enhanced Sync in General Settings.
2) Cap FPS to your monitor's Frequency. In games with variable FPS the higher the better.
Nothing I do, gives me fluent motion. There's a lot of micro stuttering both horizontal and vertical. Looks fine when I move forward and back, but when moving my mouse to change directions, it looks blurry/stuttery.
Every other game I run looks buttery smooth at 165hz (though some like Fallout 4, I had to mod to uncap without breaking the game's mechanics.)
With 40k, it feels like the game itself is actually held back by a 60fps native output.
Turning off v-sync for all games may not always be a good recommendation. I'd be dismissing the fact that some games actually work better with both v-sync and g-sync on when running a 1ms high refresh panel with frames not overlapping the limit. (Jurassic World, Apex Legends and couple others, look much smoother with both vsync + gsync on, without the noticeable input lag for me.)
I did try Fast Sync but that didn't work either. Also tried different cap values from 165 (my refresh) to 60fps. They all look bad.
As is, I can't play it for too long before my eyes begin to feel the strain.
The game looks fine on a 60hz monitor though. Maybe later I'll hook up my older 60hz panel as my third and run the game on that.

This probably a driver or config issue on your side.
I will uninstall and reinstall the game along with the latest Nvidia drivers that just showed up and see if this fixes it. :)
Also set the game to run at maximum performance in the nvidia control panel. Check your mouse settings as well. Mine was causing the game to stutter a bit until i re-tuned it to the surface i was using.
SOLVED
I figured out my problem! :)
So it would seem that this game does not work well with DSR + Gsync. I used to run this game at 4k resolutions while using my 60hz 1080p monitor, in order to gain a very smooth (no pixel edges) look. (needless to say, my old 60hz monitor is non gsync)
But here with my 165hz, I decided to run the game at my native res of 1920x1080 just out of curiosity and this time it was buttery smooth. Tried it again at 1440p and 4k (DSR), which does look nicer, but I get the micro stutters again, despite my fps counter going over 100fps. So I guess I'm stuck playing it at 1080p. No biggie.
I've read a few other games were the same way. With Doom Eternal (2020 game) I have no problems with it at either 1440p or even 4k with both G-sync and DSR. So it never crossed my mine this would be the issue.
