If you do everything right, you will get the achievement.
You choosing one of the hardest campaigns to try this achievement already tells me that you're probably making mistakes in other areas..
The people that need the achievement need to play from the beginning. They need to survive the finale. The server needs to have cheats disabled.
If the server has mods, there is a probability that the achievements are disabled.
If you follow these simple rules, you WILL (probably) get the achievement.
I recommend doing No Mercy. it's one of the best and easiest campaigns.
We usually always play with no mods well I do but I was not the host my cousin was. we all survived since I manage to get them all with a adrenaline shot on dead center but they didn’t get it and I was confused since we all were playing all the way through the campaign. But when I first got the real deal as I think it was near 5 months ago I got it just fine and I was confused on why when I played with them they didn’t get it even tho we all stood there all the way through and survived. Also I know the passing is extremely hard but the reason why we are doing it is because my cousins want to beat every campaign on expert realism. I also know as well that it doesn’t count for something to prove achievement as well but we are playing for fun and I was hoping for them to get the achievement too since my cousins were asking why they didn’t get it.
At the start of the game, type sv_cheats in your console. it should return "0" or "1". 0 means they are off and you should be good.
This is more common with achievements that have a lot of individual criterias, like the survival medals for getting X minutes on every map - These, however, are easy to fix since the survival times are saved with your Steam ID, so just loading into a map once will update the achievement's progress.
Campaign runs, however, are not saved in the same way, so a full replay may be required.
Sometimes the issue is on Steam's end, though.
Back when I was new, I had to play through Dark Carnival 5 times because it didn't want to count it for beating all campaigns on Expert, and even after the 5th time of playing through it without any slow loads or incaps/deaths, it still didn't unlock - But when I started up L4D the following day, the achievement showed as unlocked.
In my experience, this can happen if the server you're on experiences network issues, or if you're playing when Steam does their server maintenance.
I tell him to remove some of his mods since if I remember he had a error that told him he had to many pixels from a mod something related like that as he needed to restart and start up a new lobby. but for the “sv_cheats” we never messed with any of that and for us it always been sv_cheats “0” but in worse case scenario we just play a new campaign into we beat it again so they can get the achievement and also complete the whole campaign while at it and possibly get something to prove achievement as well.
I think you’re right since it was laggy and he did get a error pop up when we first began so me and my cousins might have to replay a campaign on expert realism so they can get the real deal achievement and eventually later on something to prove as they can get it as well.
So, what happened to you should be related to something else, like some glitches, or some conditions not described clearly on the achievement description. But to play safe, it's better to disable mods (but skin mods are still fine to use, I think, as skin mods usually don't contain scripts that enable cheats or something). To make sure that the achievement is obtained, try to play from the beginning & survive till the end of campaign. & also check if there's no error notification for connection on steam. If error notification shows up again (just like what happened when playing with your cousins), you'd better restart the campaign as it might make the achievement fail to be obtained again.
I use survivor mod skins when i first got the real deal achievement on my own but yeah next time I tell my cousin to disable some of his random mods he has that gives error whenever he plays a campaign having to make us reset or whole campaign or close game so we just play with no mods as it’s fine with me
