All we know, is that the most likely patient zero is an unknown man who was brought to Mercy Hospital for treatement in the ICU ward. From there, the infection likely kept spreading and afflicting more people, until a few days later it began turning them into zombie-like creatures.
It's most likely that the green flu is a naturally occurring virus that mutated from rabies, as the government wouldn't have responded so slowly if they had been the ones to create it, as they should've known it's full potential from the moment it got reported.
