Everything in the game that appears to be random is just a bunch of obscure rules picking results consistently. The only reason it seems random, is because you don't know what those rules are, or that they exist in the first place.
The item spawn rates are set for co-op and for versus with the info_map_parameters and info_map_parameters_versus entities respectively - If you made a map, you could set it so Versus doesn't ever spawn laser sights or molotovs, while setting Co-op to spawn at least 99 grenades every single playthrough.
The exact values are usually chosen by the level designer.
You can also designate specific item spawns as "must exist", which will force them to exist every single playthrough, in the exact same location.
Even the supplies dropped by uncommon infected can have their drop rates increased or decreased through map logic, and even per-difficulty, if the level designer wants to.
Left 4 Dead 1 maps ever since they were ported over the crescendo events are infinite horde untill you reach the safe room, with little to breathe break in between hordes.

I knew i wasn't imagining things, plenty of times no bile bomb was found in map 1, we've had games where not a single bile drops. I play with a few people and one of them is not allowed to ever pick up a bile in that campaign cause he uses it at the first event, sometimes he throws it away during a simple horde. I mean dude, we're playing realism expert, you can't do that crap.
