Which assuming if Whitaker didn't get infected which would be low, I can only assume he died of thirst or had to find somewhere to get clean uninfected water since as you'll see when you play the game, the store is nearly/completely empty.
Either way most of them are either infected or in the case of Whitaker, still in his gunstore.

How do you think the helicopter that save them in Dark Carnival got infected?
Or even in Crash Course.
I have no idea about dark carnival pilot though.
Then again, L4D1 was "you bite, you turn into one"
whereas L4D2 changed it.

Sacrifice comic, it explains it a bit. Everyone that comes in contact with a carrier gets infected, which is how the infection have spread so quickly.

Got bit/infected before picking up the survivors, dies after turning mid-flight, then crashing before crash course begins.
After rescuing the l4d1 cast, they proceed t oa military base of operation, which quickly gets overrun. He is shown to be in the process of becoming infected, and is promptly decapitated by a tank on page 114 of the sacrifice comic.
Most likely a civilian who's trying to help any survivors he comes across, ends up being infected by one of the l4d2 survivors (confirming that at least 1 of them is a carrier) and is shot by Nick upon turning.
That should be all the NPCs we directly or indirectly see in-game
