The helicopter crash can be seen behind the train on swamp fever.
The Passing (The bridge is blocking the way)
Dark Carnival (The car's out of gas, they escape via helicopter)
Swamp Fever (Helicopter crashed, they escape via boat Virgil)
Hard Rain (Boat is out of gas, get gas, get back to boat Virgil)
The Parish (Virgil drops you off near the bridge to get rescued via helicopter again)

In addition, it should be noted that this campaign has many errors and no end, so it is in a very beta phase and it is not known when it was finished.
It's going with the Valve exposition model where you're (ideally) not really shown or told anything, and all the information you should need to peice the story together is delivered indirectly (ie the 7 hour war newspaper clipping in Eli's hideout in HL2, just the way they refer to a 7 hour war tells you indirectly how overmatched humans were).
