Surely you meant to save a new "savegame"?!
Or did you mean to start over an new game, after having loaded the first savegame. That would destroy the efforts for gaining New Game +.
I meant start over a new game. There is no option to change difficulty before loading into a ng+ game. For that reason I'm not sure if I can get the achievements for two new difficulties.
If not, sadly I'm gonna start a new game.

Thanks for the info.
Loaded my last save with everything cleared. Changed the difficulty to Nightmare, saved the game and quit to main menu. This way my new game plus was from the start the desired difficulty. I hope it works that way.
Did you for sure get the chievo? Also do you know if you beat it on ultra nightmare you will also get the nightmare chievo?
That's interesting. But the important thing is that did you get the achievement?
f*cking awesome!!!! this way, i'll be able to play through both Nightmare and Ultra-Nightmare without having to play through them from complete scratch
