I don't think there is any revive bug. ( checks ) Nope, no reported revive bug that I could find.
What is likely happening is...
You aren't keeping a decent line of sight and solid aim on the character you are trying to revive. You just have to keep looking at the character you are reviving. The game doesn't hold you hand and automatically keep a lock on your revive target for you. You are expected to actually do your part and maintain a good sight line on the target, while attempting to revive them, and during their revive animation.
If there are a ton of enemies around, that keep getting in your way, well you will have a hard time reviving the player as they will more than likely interrupt the sight line on the revive target. A good way revive someone is get the bugs to follow you somewhere, preferably away from your revive target, and then you just run back and revive your buddy. You should also have the Revive perk on, at all times, and you should have it maxed out asap. Once maxed, every time you revive a target, all the bugs around in a certain radius will run in fear. It comes in handy.
However, if you feel it is a bug...
File a proper bug report please.

If you need help collecting this information or filling out a bug report. Let me know. I am more than happy to walk you through that process.
i know that looking around can cause this, which is not a good way of handling interactions imo... but whatever...
but what im experiencing is that even if youre as close as you possible can be, and dont move, and dont look around at all, revives STILL get interrupted sometimes.
so youre saying if an enemy limb somehow comes between the character models of the other player and my player, and interrupts the direct line of sight, this can interrupt the revive process?
this probably happens easily since theres a lot of short distance clipping.
but even this doesnt explain the revive interruptions ive experienced... it even happens without any enemies around. im not sure if its related to network issues maybe?
if i had to guess, its related to the terrain physics though...
its not too uncommon to find a downed player that is glitching around on the map, stuttering in place... and whenever this happens, its quite often hard to revive them since their character model seems to actually be moving/suttering around on the map. so i suspect sometimes, despite the character model not moving and stuttering, the way the game detects the location of the downed player is still glitching and jittering around sometimes.
all it takes is a fraction of a second for the game to misinterpret the location of the downed player, or the player reviving him, which interrupts the revive process.
Could be. Again, I can't say for certain but, I have run into situations like that before and usually the reason it stops mid revive is because I look away by mistake or some teammate jumps on my head or something is causing me to move around and it breaks the revive animation.
I am fine with it thought. It's all part of the fun. Lol.
In the good times, and the bad times. Lol.
There are issues right now with the level geometry. People have reported falling through levels and stuff even though there is solid ground in front of them. This usually happens when there is terrain deformation happening near by. Ex. Someone blows up a grenade that takes a few chunks out of the map, Satchel charges going off, a fast fall and dying before hitting the floor, etc.
I think there are already a couple bug reports about that stuff. I will need to look again. I know someone else has reported issues like that and it wasn't more than a day or two ago. He probably made a bug report.
All in all, I wouldn't worry to much about it and just enjoy the game. There is an update about to hit and it should fix a lot of the issues that people have been having.
The closest one I could find was this.
https://ghostshipgames.atlassian.net/browse/DRG-787?jql=text%20~%20%22Falling%20through%20level%22
Although I am still looking.
If there isn't one, considering making the bug report following the instructions I gave you along with the Template GSG provided us in a pinned post.
but im pretty sure ive been into situations without an enemy around, and revives still failed, sometimes several times in a row... and without looking around at all or moving
ive clipped and glitched through terrain more than usual lately haha
idk, yeah i still enjoy the game, but... id really prefer if the game was more solid.
9/10 times it doesnt matter, but this one time where the whole team gets downed and you manage to save the game... except you dont, cause the revive fails, and maybe even a second time... its really annoying... and even when you manage to save the match, despite the failed revives, its annoying and makes it less fun... for me at least :P
Again, if you really think it's bugged. Report it.

Let me know if you need any help.
You know never, they might have taken care of it.
I haven't checked new update, but yes, the game should not do this. Once you start revive animation, since you are locked in place, target should not be able to move out of range.
Have you made a bug report about? Would you like me to make one for you guys?
Let me know.
Strange as it is working for me.
Mind you the bugs need to be close enough to be affected by the perk's effects.
