Would it really require that much of an increase, if any? They actively drink, promote drinking by putting buffs associated with it, and you can even play drunk.

I would have thought this alone would put them well within the rating requirement to put smoking in the game.

edit: To further clarify a bit, the question was answered in a dev stream sometime after the 1.0 update, possibly late 2020-early 2021 (I personally don't remember), and they did not give a direct reason for changing their minds, only hemmed and hawed, but did state it was no longer being added to the game
The entire reasoning could easily be interpreted as something ratings-related
