If yes, I can reproduce this Error on my Pc, therefore my Question

My Reverb G2 (in my Example) were found, but my HMD and the HoloLens Sensor is deactivated and offline. The only thing i have to do to fix this SteamVr failed initialization is to activate the Driver for my G2 (where I have disabled for the example to show you better ) . If i do this than all works fine

Therefore make sure your HMD is activated (not just shown as "offline" or deactivated in your Device Manager)

(i know you dont understand German, but you know this Screen in your Windows Settings do you ?. The Text highlighted in yellow just means allow access to the camera and allow Apps to access the camera. Make sure it is checked.
In my Opinion it is just an Driver Issue with your HMD. A few tips (Example) I have told you now to fix it. Good Luck and give a Response if you fixed it.
