Report a translation problem
Are you doing anything different that you normally would when you play VR? Perhaps playing with the lights out for the ambiance of someone watching? You need a fair amount of light for the Rift S tracking.
Mainly though you just might just have to work around the headsets tracking limitations and be mindful of how you hold your hands so that the headset doesn't lose your controller tracking.
Also you might wanna check reddit out for optimal tracking with the Rift S, there are some tips out there.
The weird thing is that if I try to cover up one of my controllers with the other hand on purpose it is still working.
It only breaks if I am doing one of the mentioned actions.

The first clip shows how the hands are lagging but as soon as I enter pause menu they are fine again. This only happens if I am at low health.
Funny thing is, if open the Oculus menu to see the virtual desktop, the hands move fine there as well.
The second clip shows how the hands start spazzing around as soon as I get attacked and it takes some time until I get full control back.
My hands are not even out of sight or overlapping each other. And as soon as I dont recieve damage anymore I can move my hands properly again.
Third clip shows how weird the hand moves when I use the syringe.
For the last clip I tried out Boneworks to see how it is in there. The gun also moves in weird ways as soon as I try to use it with both hands. Even weirder is that the tracking is not the only problem. Sometimes the gun keeps firing even though I released the trigger already.
I am playing in a well lit room, have the latest drivers and I am using a 3.1 usb port.
Apparently the only thing I can try is to remove the cables once and plug it back in.
Bought Alyx today and I already experienced loss of tracking when I mainly played Beat Saber, but I thought I fixed them but disabled power saving and thingies.
Not the same issue as yours but it's still a tracking issue, I end up having my hands stuck near my "chest" and can't do anything except rotations. This isn't new from the Oculus Rift base but that's annoying. I don't mind having to restart Oculus services after a session of Beat Saber, but 30min in HL Alyx and my immesion is broken since I have to quit the game and restart all of this.
Also using a Rift S so... Either something is wrong with USB power management in Windows (but I've disabled every power saving function), or the cameras are really bad when your hands get close to you.
I fixed it but I am not sure what exactly was the reason. I did following things:
Switched USB and Display Port multiple times.
Unplugged the cable from the headset.
Maybe it works if you completely unplug the cables from the pc and the headset for a few seconds and then reconnect it.
Switching display port also helps with flashing pixels
