Is the yellow lungs warning perminant?

I am 80% sure that your lungs heal at least *somewhat*. If you mistreat them further, they can also go red. Both are not digital health steps, instead those icons are a digital representation of their analog health state.

You can eat the white pill to fully heal the lungs (and yourself) and can produce new ones with the Chemistry Station plus 1 silver plus 1 cobalt plus I think 8 fern. And the following was true at least until the Terrain Update came out: You could also just create the Cryo Chamber, hook up the gas pipe to the room's air (Careful with the placement of the Pipe Cowl, if you use that one.), power it, turn it on, strip, and enter it. You go unconscious, but you can exit at any time with "q". The chamber will also heal you, so no more pills required.

The yellow/red lung warning can also be a temporary indicator of a current situation: Bad air mix/pressure, something like that. What you breathe must be between 0 and 50°C, and the oxygen partial pressure must be at least 20 kPa. And if you're in a closed spacesuit right now, consider flushing the suit via the button in the helmet, because maybe your oxygen isn't pure and you don't have the right filter (e.g. nitrogen), so maybe your suit built up something that prevents further oxygen from coming in.
Well I already used the pill prior. It's still showing a day later. Guess see what happens.
If you're standing in a room with helmet open, most of the air is oxygen, pressure is say 40 kPa, temperature is ~25°C, there's basically no pollutant/volatiles/N2O ... then it's permanent (healable) lung damage. If however your suit is closed, it may be a suit situation that you could (and absolutely should) deal with.
