Maybe the leak allowed toxins (X / Pollutant) into your suit and you didn't flush them out (nor had a filter to filter it out).
Maybe you played on Europa or Vulcan and your lungs got frozen or burned, making you enable to breath at all.
It's also possible that you accidentally turned off one of your suit functions.
And last but not least, non of the above applied, but the air in your oxygen tank was so warm or cold that it your suit couldn't correct the temperature. When that happens it tries to fix everything by pumping the air to your waste tank and pull fresh air from your oxygen tank (and repeat the cycle). This could cause your lungs to Burn / Freeze eventually, if not drain the oxygen.
I seriously thought I was bugged the first few times that happened to me, but since I pay attention to the temperature of the oxygen tank all is well.
Maybe you run out of electricity or your space suite was destroyed.
When bad things happen sometimes we dont notise things while we desperate try to repair our suite.
Keep in mind you start in an EVA suite designed for space, not for planet operations.. so its rather fragile.
Anyway, lets image you rip your suite and lost allot of presure and heat, then your suite have to use allot of battery power to compensate = you use electricty much faster than normal = so if your battery was low and this happen there is a chance you did not notise you had no more electricty due to x alarms.
When you run out of electricty your space suite stop working and then you die pretty fast no matter if you repair your space suite or not.
No electricty = death within xx seconds.
As Lugard write, maybe you damage your lungs due to extreeme cold/heat or high/low presure.
nope, fixed everything, helmet and suit. No scratches anywhere.
Checked co2 filters, they where ok, waste and oxygen tanks were ok, temperature was low but rising, battery at medium charge.
Oh well..it happened, i reloaded, no big deal ;)
