[No Spoilers] Anything easy to miss?
Anything to know about before I start? I don't generally replay games, so want to get the most out of one playthrough (without directly using a walkthrough or something silly like that). Is there anything I *must* know about?

There are a lot of hidden quests. Generally, if your main objective is to go to X, go everywhere but X first and you'll probably discover something interesting (at the very least, NPC plotlines will progress, and they're all fun) along the way.

I recommend you consult a spoiler-free guide if you're concerned about missables.
speaking of hidden missions, they are typically given by non-main characters. Some are, but not all. Also, most hidden quests take lace in the afternoon after a Day worth of usual activities. Im not 100% sure atm but I think if you use your Map button, you can look for a Green ! icon if there is a hidden quest in the area you are at

You need to play this game at least twice to get every achievement.

You also need a walkthrough otherwise you will be missing on hidden quests and some AP bonuses.

Hidden quest does NOT show up on the map.

Nothing really important to miss. You can find many of the secret quests without any guide as long as you talk with most NPCs, but then again the rewards aren't something that special in most cases.

Thanks all. Seems like I'll be able to get 90% by just taking my time and talking to people without using a walkthrough (even a spoiler-free one), which is good.

Yeah, if you're willing to talk to everyone twice after each story beat, you should be able to get the stuff no problem.

This game is a little less obnoxious about it than Trails in the Sky was, if only because this game doesn't have guard posts at opposite ends of each region which are really tempting places for sneaky developers to put in collectable books.

Make sure to talk to each NPC at least twice, since some hidden quests are only available the second time you talk to them.

And make sure to buy books from the bookstore to complete your book note. Buy accessories that are part of a set and hold on to them. Do fishing at every opportunity and exchange points for accessories and a master quartz. You need them all for ultimate weapons.

"Twice" really means "Until the dialog starts repeating". In Sky, at least, there were times when it took more than two times to get them to run out of dialog. Can't remember if there was any in Cold Steel.

I think the only thing you must want to plan is with who you want to forge Strong bond, it's only thing you can fail, because it requires specific actions. Well you technicaly can miss Moon Rose books and also collect all items that part of "sets" for end game secret. But books are really easy to collect, compared to sky series.

Missed books can be bought from the shop as rarities, although they show up randomly and may require a lot of entering and exiting to show up. The biggest thing is just missing getting S ranks on every chapter and the bonuses that are handed out as a consequence.

It's unlikely, but certainly not impossible you could miss out on getting the medals if you don't talk to everyone and scan as many bosses as possible. Buy a lot of the scan item so you don't have to worry about keeping analyze on someone when you don't have Elliot or Emma in the party.

Missing last rank is also seems unlikely to me, no unique bonus points as far as i can remember, especially compared to some REALLY hidden conditions in sky, and the amount of handicap points is huge aswell. But reading about points is basically killing all the fun from a game and spoiling alot, especially since there nothing that hidden.

Well you need to finish game at least twice to get achievments, and by that i mean ALOT of achievments require two PT's.
