The DLCs add additional items, some of which can be unlocked by reaching 2 or 3 star hospitals. Most are mainly for decorative purposes to boost your hospital's attractiveness but some add to gameplay like robot janitors, increasing comfort or reducing boredom. While gameplay is similar for all DLCs, they do add some variations due to different challenges. You might want to watch some playthrough vids to get an idea of what DLCs you might like.

All hospitals in the top part of the list up to Croquembouche in the urban region are in the base game. The rest are from the DLCs. If you're getting the bundle, I'd recommend first going for 3 stars on all hospitals in the base game that unlock items for reaching that level as some of them can help you in the DLCs.
